Advanced Illness Management
Improving quality of life for patients with complex conditions
Vituity Family Medicine Center offers Advanced Illness Management, a palliative care program designed to augment other medical treatments focusing on quality of life. Our care is individualized and includes high levels of support to help patients with serious illness coordinate care with their specialists and determine the right path for their physical, mental, and spiritual health. Our goal is to reduce suffering and improve the quality of life for both patients and their families.

The Advanced Illness Management is an outpatient program that provides:
- Quality of life and end-of-life care
- Easy access to providers with palliative care training
- A dedicated call line to answer questions and concerns quickly
- Licensed social worker
- Quick access for prescription refills
- Home service referrals
- Chaplain services
Advanced Illness Management treats people who are suffering from serious illnesses such as cancer, congestive heart failure, dementia, liver disease, Parkinson’s/ALS/MS, COPD, stroke and many more. Patients who require considerable assistance, require frequent medical care, or have multiple chronic and/or co-morbid medical conditions can also be treated through Advanced Illness Management.
The doctors and staff can help coordinate care with frequent communication back to other care providers. Patient needs are addressed as soon as possible so that we provide extra care when patients and families need it most.

Araz Melkonian, MD | Associate Medical Director
Medical School: Al-Nahrain University, School of Medicine
Residency Program: Adventist Health Glendale
Fellowship Program: University of Southern California
Dr. Melkonian is board certified Family Physician and Geriatrics Medicine specialist. Dr. Melkonian has extensive training and experience in palliative care and hospice medicine.
Dr. Melkonian has been leading the Inpatient Palliative Care Program in AHGL hospital since 2021. He is currently practicing the full spectrum of family medicine, focusing primarily on managing patients with serious illnesses and improving the quality of life and functional ability of the elderly population and patients with serious illnesses.
Dr. Melkonian is committed to leading the Advanced Illness Management team to provide the right tools and support to our patients and families, by addressing their physical, psychological, social, and spiritual needs.
Molly Wilkerson, MD
Medical School: Keck School of Medicine at USC
Residency Program: Adventist Health Glendale
Dr. Wilkerson completed residency training in family medicine at Adventist Health Glendale and was delighted to join the faculty in the fall of 2022.
She is currently practicing in family medicine but is passionate about providing care for patients with serious illnesses. Dr. Wilkerson’s emphasis is in geriatrics and palliative care. Her goal as a physician is to empower her patients to make medical decisions that align with their values and goals for their lives.
