Thank you for entrusting your health to the physicians and providers at Vituity Family Medicine Center and Adventist Health.
We are writing to let you know that the clinic is now part of Adventist Health, operating as Adventist Health Family Medicine as of April 1, 2025.

This transition is a strengthening of the partnership we have had with Adventist Health for many years, as the resident
physicians working in our clinic are part of the Adventist Health Glendale Family Medicine Residency program.

What this Means For You

Same Trusted Providers
You will continue to receive the same high-quality care from the same doctors and providers at the clinic at 1560 E. Chevy Chase Drive, Suite 355, in Glendale.

No Disruptions
There will be no need to transfer your medical records or change your patient portal account to continue seeing your current providers.

Familiar Access
Clinic location, hours, and primary care services will remain the same.

Communication Changes
After April 1, 2025, you will begin receiving communications and bills from
Adventist Health.

Insurance Coverage (Action Required)
It is important that you contact your insurance provider to confirm that your health plan is contracted with Adventist Health Physician Network.
